Please note: There is a $35 nonrefundable registration fee per child per summer term.
A risk and consent form must be signed and filed for each student prior to the start of the program. Download this form now (PDF). (This form will also be mailed to you upon registration.)
You may withdraw via telephone or a written request. If the request is received prior to the refund deadline, you will receive a 100 percent refund, less the $35 nonrefundable registration fee. Fees are not refunded after the deadline. A complete refund will be made if the session has been canceled by UW Extension.
