Selection Forms:

Teachers- you must print paper copies of the selection forms and have them completed by each registered student and teacher. Staple Forms 1 and 2 for each participant (or have them copied front to back) and mail the completed copies back to us immediately.

Do not complete selection forms for students and teachers who are not registered, unless someone among those registered cannot attend, in which case you may substitute a new participant.

Instructions:

Please note that there are two selection forms. Each registered student and teacher should make his or her selections on both forms.

• Selection Form 1 (general sessions)
• Selection Form 2 (UW language classes)

Students must:

• select 4 presentation choices per hour, in order of preference (1 for first choice, etc.) (Form 1)
• select 2 language classes to observe (Form 2). Space permitting, students will be scheduled to visit one language class between 9:30 a.m. and 2:20 p.m.
• schedule 1 open hour for lunch and do not mark selections for that hour
• note name, school and language interest on BOTH forms

• select 4 presentation choices per hour, in order of preference (1 for first choice, etc.) (Form 1)
• schedule lunch for 11:30, as a complimentary teacher lunch is provided
• note your name, school and language interest on BOTH forms
